‘Thyroid surgery using oral endoscopy reduces swallowing difficulties’

A research team of Seoul National University (SNU) Boramae Medical Center reported that thyroid surgery through an oral endoscope reduces the risk of developing post-surgical swallowing difficulties (dysphagia) compared to existing operations with an incision.

The research team surveyed and conducted a voice test on 100 patients who received thyroid surgery at the SNU Boramae Medical Center and followed up six months after surgery.

Researchers found that patients who underwent surgery through an oral endoscope developed fewer dysphagia symptoms while showing similar performance to those who underwent operation with an incision.

The research team confirmed that all of the patients' voice functions recovered to their preoperative level after three months of surgery. The three-month postoperative satisfaction rate was higher in patients who received operation through an oral endoscope.

Patients develop dysphagia symptoms when tissues around the thyroid gland form adhesion after surgery and pull the skin along the esophagus and airways when food is swallowed. Dysphagia is challenging to be improved, and it is a major cause that lowers patients' life quality.

Thyroid operation using an oral endoscope is being adopted worldwide as it leaves no scar, unlike the existing surgery, and it costs the same to the conventional surgical method, the hospital said.

"Dysphagia complications occur when the incision site contracts during the healing process and cause tissue to adhere together after receiving surgery with incision," said Professor Chai Young-jun of SNU-Boramae Medical Center. “Operating with an oral endoscope is also functional because it does not cause skin contraction at the incision site and reduce the risk of complications."

The study, "Functional Voice and Swallowing Outcome Analysis After Thyroid Lobectomy: Transoral Endoscopic Vestibular Versus Open Approach," was published in the World Journal of Surgery in August 2020.
